G = Good (condition of monument code)
P = Poor, disturbed monument, or altered position
N = Searched for but not found (add details in COMMENTS column).
D = Destroyed (submit disk or proof station has been destroyed; otherwise station should be coded 'N').
GPS
Can station be used for GPS observations? YES means no visual obstructions extending higher than 15 degrees above the horizon
OK = At least two quadrants with no visual obstructions extending higher than 15 degrees above the horizon and no visual obstruction extending higher
than 30 degrees above the horizon.
NO = If any visual obstruction extends higher than 30 degrees above the horizon in any quadrant.

PID
The six-character NGS Permanent Identifier. If the PID is not known, enter the disk INSCRIPTION in the PID column (i.e. the agency name cast in the disk;
e.g., CGS for US Coast & Geodetic Survey, NGS for National Geodetic Survey, USGS for US Geological Survey, USE for US Army Corps of Engineers, WIDOT
for Wisconsin Dept. of Transportation
Designation
The station name. If the station name in not known, enter the disk stamping. When there are designation or inscription questions, please make and
submit a rubbing.
Condition
Other location or condition information. If a PID is not listed, enter either approximate latitude-longitude, local reference information, or plot on a
copy of a large-scale map.
